Magnolia Hospice Austin - Nurse Practitioner
General Purpose: To supplement hospice physician/Nurse Practitioner/Nurse Practitioner services and/or in the absence of the Hospice Medical Director, may act as the physician/Nurse Practitioner representative of the Interdisciplinary Group (IDG), providing medical direction, including the care provided in the home, general inpatient care, emergency services and respite care related to the terminal condition. May be responsible for the hospice patient’s medical care and act as the primary medical resource to the IDG, providing medical coverage and support during and after normal business hours, as well as making visits to patients for medical assessment and intervention whether in a facility or the home and completing required Face-to-Face visits.
ESSENTIAL FUNCTIONS
- Supervise the planning, coordination, and delivery of the Face-to-Face encounters.
- Participate and provide expertise to the team at regular interdisciplinary group meetings, the development, review and explanation of Face-to-Face findings when necessary.
- Monitor compliance with policies and procedures.
- Communicate with the Medical Director regarding Face-to-Face encounters, the need for further evaluation and/or second opinion(s) when necessary.
- Provide constructive feedback of support and clinical staff in cooperation with other members of the management team.
- When appropriate, provide information related to written performance evaluations for all staff in cooperation with appropriate members of the management team.
- Identify the need for staff education and assist in the development of in-service programs for interdisciplinary group.
- Assist in education/training programs.
- Assist with pre-employment screenings, testing and physicals as needed.
- Perform specific assignments as directed by the Director.
- Assesses the nursing needs of the Hospice patient at the time of the Face-to-Face encounter.
- Answer questions of the Hospice patient and/or family members about providing care for the patient.
- Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
- Always communicate with the Director and Medical Director if unable to meet a scheduled Face-to-Face encounter.
- Maintain skills and knowledge.
- Participates in QAPI program.
JOB FACTORS
Physical Requirements: Requires moderate physical effort to meet, converse with and access active patient(s) receiving hospice care. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.
Mental Requirements: Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working Conditions: Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous.
Transportation: Must have a current valid driver's license, auto liability insurance and reliable transportation.
